STEP 1 – Contact Us

Once you have decided to pursue an Age Management Medicine program, simply complete the Cenegenics Patient Information Sheet and FAX to 678.623-0041.

STEP 2 – Blood Test

Dr. Smith will arrange a blood draw for your initial laboratory evaluation at your home or office.

You must be fasting for 8-12 hours before the test is performed - nothing to eat or drink except water.

STEP 3 – Medical History Forms

Complete and return the Medical History Form while you are waiting for your lab results (this form will be emailed to you).

STEP 4 – Executive Health Evaluation

Once I have your lab results and your completed Medical History Form we will schedule your Executive Health Evaluation.

This comprehensive 5 hour evaluation will include physical exam, medical history review, lab panel review, bone density and body composition scans, exercise and nutrition assessments, and treatment planning.

STEP 5 - Treatment

At the end of your EHE, you and your physician will determine the course of treatment. Should you opt for treatment your medications and supplements will be shipped to you the next day and every 28 days thereafter from our pharmacy.

STEP 6 – Follow Up

After 8 weeks of treatment, you will have your first follow-up lab work. Lab draws are typically done quarterly from this point forward.
